Access GP21A forms

GP21A forms are used to notify us of the appointment, termination or change in status of a contractor or assistant.
These forms are only to be used by NHS boards and will only be accepted from authorised signatories at each NHS board.

There are three GP21A forms you can access and download:
- Part 1 Sub-part A
- Part 1 Sub-part B
- Part 2
How to submit forms
Once the form has been completed, the authorised signatory should submit it to NSS.psd-customer-admin@nhs.scot from their personal NHS email account, with 'GP21A' in the subject field.
Please note that paper forms will not be accepted.
